About the Personal Health Coaching Program
The Personal Health Coaching Program, provided by the OSU Health Plan through Your Plan for Health, is staffed by The Ohio State Medical Center health care professionals experienced in leading personal health and wellness strategies in:
- Weight management
- Nutrition
- Exercise
- Stress management
- Tobacco cessation
- Blood pressure, cholesterol and diabetes management
- Disease prevention and management
Our goal is to help you address your personal health and wellness and achieve your goals. We have coaches with a range of clinical and behavioral backgrounds, including exercise physiologists, social workers, health educators, tobacco cessation specialists and dieticians. Each coach is trained in all areas to promote a holistic approach to meeting your personal health needs. Program participants appreciate the support, awareness and accountability with their personal health coach which helps keep them on track for reaching their optimal health.
What Is a Health Coaching Session Like?
Our coaching sessions take place by telephone and consist of 15-30 minute conversations between you and your personal health coach to help you set goals to improve your health and wellness. During your initial appointment, session dates and times will be planned at customized intervals based on your goals and support needs. Your coach will provide education related to your health concerns, answer your questions, make any necessary referrals, and keep you motivated.

Our Services:
Support: We support you while you are learning to master managing your health. We help you in areas where you may feel stuck, or lost motivation and just need the support of someone to help get back on track.
Guidance: We guide you to take charge of your own health with personalization of your health coaching program to meet your needs.
Education: We teach you a preventive, self-care method and process that you can use and adapt to your own situation in life. We provide you with updated health information on topics such as nutrition, physical activity, weight management, stress management, or tobacco cessation upon your request.
Connection to Resources: We also link you to resources on campus and in the local community that support you while working on your goals.
