Are You Eligible?

All benefits-eligible Ohio State faculty, staff, and medical-plan enrolled adult dependents who complete their Personal Health Assessment (PHA) and consent to release their results to the OSU Health Plan are eligible for health coaching services. Participants should not have any underlying chronic condition or disease. If your PHA indicates a high risk for one of our healthy lifestyle improvement areas, you will be contacted and invited to participate in the program. If no areas are indicated for healthy lifestyle improvement risks in your PHA, but you would still like to receive personal coaching in one of the health areas provided through the program, you may self refer by contacting us at (614) 292-4700 or toll free at (800) 678-6269.

If you are living with a chronic condition – such as asthma, diabetes, chronic obstructive pulmonary disease (COPD), or heart disease – and are enrolled in a university-sponsored medical plan, you are eligible for the Your Plan for Health (YP4H) Care Coordination Program. This service, provided by OSU Health Plan, can help you better manage your condition to improve your health, make you feel better, and even save you money. To begin your Care Coordination experience, call the OSU Health Plan at (614)292-4700 or call toll free at (800) 678-6269 – Monday through Friday from 8 a.m. to 5 p.m.; choose option “0” for customer service.
