Your Plan for Health

Your Plan for Health (YP4H) is Ohio State’s preventive health care program focused on providing tools and resources to empower employees to become proactive in managing their health.

YP4H offers resources and incentives to help faculty and staff reach their goals and maintain healthy behaviors. YP4H tools and resources including free annual biometric health screenings; online Personal Health Assessment (PHA) questionnaire (encouraged annually); health/lifestyle management programs; health coaching and care coordination for those with chronic conditions, such as asthma, diabetes, COPD and heart disease. The services and resources are offered free of charge to employees of The Ohio State University who participate in YP4H.

In addition, YP4H offers seasonal flu vaccinations, Lunch and Learn workshops, and a variety of health fairs and educational opportunities to help Ohio State employees be well and enjoy healthy lifestyle behaviors – and all are free of charge.

The Ohio State Employee Assistance Program (EAP)

The Ohio State Employee Assistance Program (EAP) offers Ohio State faculty and staff confidential life assistance services, including consultation, assessment and referrals, brief counseling, progress monitoring, after-treatment review, and education/training. Relationship and workplace issues represent the largest volume of requests for EAP services, but EAP is able to address other problems, such as:
