SPOUSE AND SAME-SEX DOMESTIC PARTNER BIOMETRIC HEALTH SCREENING SITES

Free biometric health screenings are now open to spouses and same-sex domestic partners enrolled in a university medical plan. You are eligible to receive one free screening per calendar year. To schedule your appointment, follow the instructions below:

• To search for available appointments at a specific location, click on the "All Locations" tab and select a location.
• To search for available appointments on a specific day, click on a day within the "Appointments" calendar. When you click "register" you will see the open appointment times and locations.
• To search for a specific location on a specific day, click on "Register", then select a location from the "Locations" drop-down list.

Be sure to bring to your appointment your photo I.D. (driver’s license), as well as the OSU employee's eight- or nine-digit Employee Identification Number (found on a pay stub or the Buck ID). You also will need to show your medical insurance card.
