BIOMETRIC HEALTH SCREENINGS

Biometric health screenings (measuring blood pressure, cholesterol, glucose, weight and height) are available free of charge only to faculty and staff of The Ohio State University.

These health screenings, which are recommended annually, can be used to obtain the health data needed to complete the Personal Health Assessment (PHA), the cornerstone of the university's Your Plan for Health initiative. You may schedule your free health screening at anytime during the calendar year. You do not have to wait for the anniversary of your last screening to take one this year.

Free biometric health screenings are now open to spouses and same-sex domestic partners enrolled in a university medical plan. Click on the “Spouse and Same-Sex Domestic Partner Appointment Calendar” link below and follow the instructions to schedule your appointment.

To schedule your screening:

1. Visit the Faculty and Staff Appointment Calendar, or the Spouse and Same-Sex Domestic Partner Appointment Calendar and choose the date that works best for you. Click "Register" to see all open appointment times and locations for that date.

2. To search for a specific location, select “Register” on a specific date, then "Locations" to select your preferred location. No appointment times will appear if the location you choose is not sponsoring screenings on that date. You should receive a confirmation e-mail of your appointment. If you do not receive e-mail confirmation, please call (614) 292-1894.

3. If you need to cancel or change a previously scheduled appointment, e-mail bewell@osu.edu or call (614) 292-1894 to cancel your original appointment before trying to reschedule.

Please commit to the screening appointment you have selected or cancel as early as possible to ensure appointments are available to all your coworkers who want one.

The day of your screening:

  • Fasting (going without food) for 12 hours prior to your screening appointment is recommended to obtain the most accurate results.
  • Drink plenty of water during the 12 hour fasting period and at least one 8-ounce glass of water one hour prior to your screening appointment.
  • Since screening appointments are scheduled back-to-back please be sure to arrive on time, allowing approximately 20 minutes for your appointment. Please be advised that if you arrive more than five minutes late for your screening appointment you may be asked to reschedule. The screening will consist of a finger stick to determine your cholesterol and glucose readings, as well as measurements of your blood pressure, height and weight.
  • Your health screening results will automatically be entered into your Personal Health Assessment file during the screening. You also will receive a copy to share with your physician.
  • FACULTY AND STAFF: Be sure to bring your BuckID, hospital ID or other photo ID; your 8-digit employee ID number (located on BuckIDs issued after 2002 or your pay stub.)
  • SPOUSE AND SAME-SEX DOMESTIC PARTNER SCREENINGS: Please bring to your appointment your photo I.D. (driver’s license), as well as the OSU employee's eight- or nine-digit Employee Identification Number (found on a pay stub or the Buck ID). You also will need to show your medical insurance card.

This website is available only for scheduling health screenings. You may access the Personal Health Assessment at www.yourplanforhealth.com.
