Personal Health Assessment (PHA) SCREENING SITES

• To search for available appointments at a specific location, click on the "All Locations" tab and select a location.
• To search for available appointments on a specific day, click on a day within the "Appointments" calendar. When you click "register" you will see the open appointment times and locations.
• To search for a specific location on a specific day, click on "Register", then select a location from the "Locations" drop-down list.

The following health screening values are needed to complete the Personal Health Assessment (PHA): height, weight, waist circumference, resting pulse rate, body mass index (BMI), blood glucose, total cholesterol, LDL, HDL, triglycerides, and blood pressure. If you have been screened in a physician’s office in the last 12 months , you can input the values you received from your physician into your PHA questionnaire. A campus screening is not required to complete the PHA if you have obtained the necessary biometric health data from a different source.
